Long-Term Care

Quality Assurance and Evaluation Studies for Home and Community-Based Medicaid Long-Term Care Services (Senior Initiatives)
The Robert Wood Johnson Foundation and Centers for Medicare and Medicaid Services, U.S. Department of Health and Human Services
A series of analytic activities to help the NJ Department of Health and Senior Services assess the implementation of Medicaid-based programs designed to provide seniors and their caregivers with a broader range of home and community-based long-term care options, including: Adult Day Health Services, New Jersey's Nursing Home Transition Program, Assisted Living, and the Caregive Assistance Program.

Nursing Delegation and Medication for Elders in Assisted Living
American Nurses Foundation
Through a survey of state nursing boards, key informant interviews, and focus groups with assisted living nurses and administrators, CSHP is identifying current state policies on nursing delegation of medication administration for elders in assisted living. This project will also identify the clinical implications of the policies as well as lay the foundation for a program of gerontological research in nursing delegation of medication administration for elders in assisted living settings.

Community Living Exchange Collaborative
Centers for Medicare and Medicaid Services, U.S. Department of Health and Human Services
In partnership with a broad array of organizations and stakeholders, CSHP is providing training, direct technical assistance, information collection and dissemination, resource development, and other assistance to state agencies and other recipients of Systems Change Grants for Community Living from the Centers for Medicare and Medicaid Services. This technical assistance is designed to assist these states in implementing programs and policies to develop and enhance independent living opportunities in the community for people with disabilities.

New Jersey Health Initiatives Workforce Agenda Evaluation
The Robert Wood Johnson Foundation's New Jersey Health Initiatives

The New Jersey Health Initiatives (NJHI) competitive grants program supports the development of programs designed to improve health and health care in New Jersey. The NJHI Workforce Agenda addresses shortages in two segments of the health care workforce: (1) registered nurses in the acute care hospital setting and (2) paraprofessionals in the long-term-care institutions and home health care settings. Grantees for this Program will be selected in fall 2003.

The Center for State Health Policy's support for the goals of the Program includes four components:
• Environmental Scan-An ongoing scan of the state policy environment and labor market trends relevant to the Program to help NJHI and grantees achieve their goals.
• Grantee Technical Assistance-Technical assistance to help grantees establish outcome objectives and practical, useful self-evaluation activities
• Program Assessment- A process evaluation of all grantees to document their activities and barriers to and facilitators of success.
• In-Depth Evaluation-An in-depth outcome evaluation of selected grant sites or clusters of sites.

Access to Health Care

Sustaining Individual Health Insurance Markets Under Community Rating and Open Enrollment
The Commonwealth Fund and The Robert Wood Johnson Foundation

This study is assessing the reasons that enrollment in New Jersey’s Individual Health Coverage Program (IHCP) has declined and an analysis of policy options for sustaining coverage in this market. Data collection for this project is complete; analyses are currently underway. Conducted in cooperation with the New Jersey Department of Banking and Insurance's Individual Health Coverage Program Board.

New Brunswick Community Health Assessment
New Brunswick Tomorrow (with funding from Johnson & Johnson)
This joint project with the University of Medicine and Dentistry of New Jersey’s Robert Wood Johnson Medical School is engaging the leadership in the greater New Brunswick community to identify and address health problems that affect this community, and contribute to the developing health improvement programs through the Healthier New Brunswick 2010 initiative. A survey of 600 New Brunswick families will go into the field this winter. A series of focus group interviews with key informants, which informed the survey design, have been completed.

Survey of Experience with Mandated Medicaid Managed Care Enrollment
New Jersey Developmental Disabilities Council
With the onset of mandatory Medicaid managed care enrollment for Supplemental Security Income (SSI) recipients in New Jersey, the New Jersey State Council on Developmental Disabilities asked CSHP to survey recent enrollees to assess the enrollment process and initial Medicaid managed care experiences of persons with disabilities. Survey data collection and analyses are completed; a report will be compiled in Fall 2003.

State Solutions: An Initiative to Improve Enrollment in Medicare Savings Programs
The Robert Wood Johnson Foundation and The Commonwealth Fund

CSHP serves as the National Program Office for the Robert Wood Johnson Foundation initiative to increase the enrollment of low-income older adults in the Medicare Savings Programs. In this capacity, CSHP will co-ordinate technical assistance to five states that will receive grant support from The Robert Wood Johnson Foundation for this pilot program. Additional funding for this effort is provided through The Commonwealth Fund.

Pilot Study of a State Policy to Reduce Racial and Ethnic Disparities in Cardiac Care
Agency for Healthcare Research and Quality, U.S. Department of Health and Human Services

New Jersey State Planning Grant on Health Coverage
The Health Resources and Services Administration of the U.S. Department of Health and Human Services has awarded the New Jersey Department of Human Services a one-year, $982,075 grant to study the problem of the medically uninsured in New Jersey and identify promising approaches for addressing gaps in coverage. The grant provides support for a series of studies of state coverage programs and the uninsured. These studies will be carried out by the CSHP.

New Jersey State Planning Grant
Health Resources & Services Administration of the U.S. Department of Health & Human Services

In July 2002, the Health Resources & Services Administration of the U.S. Department of Health & Human Services, awarded the State of New Jersey a one-year State Planning Grant to examine the issue of the uninsured. The New Jersey Department of Human Services (DHS) was designated lead agency for this grant and is collaborating with the Center for State Health Policy (CSHP) on six analytic research projects which include:

Optimizing NJ's Premium Support Program-New Jersey's Premium Support Program (PSP) subsidizes employer-sponsored health insurance coverage for FamilyCare eligible persons who are employed and offered employer-sponsored coverage that meets specific requirements. CSHP is examining policy options to increase PSP membership while preserving cost effectiveness
Strategies to Improve Enrollment & Retention in NJ FamilyCare (Part 1)-
CSHP is conducting and analyzing a survey of 650 families with members currently enrolled and recently disenrolled from the NJ FamilyCare program in order to assess patterns of selection into and out of NJ FamilyCare.
Strategies to Improve Retention in NJ FamilyCare (Part 2)-CSHP is conducting two longitudinal analyses of program enrollment data for Medicaid, NJ KidCare and NJ FamilyCare, including updating an analysis of program retention that the Center performed for NJ KidCare for January 1998-April 2000 and examining the likelihood of successful transition from Medicaid to NJ FamilyCare.
Convene an Expert Panel on State Health Insurance Regulations-
In April 2003, together with the NJDHS and the NJ Department of Banking & Insurance, the Center convened an Expert Panel to assess the existing body of research on insurance reform and draw implications for current reform efforts in New Jersey. Panelists included leading researchers, health insurance industry experts, consumer advocates and policymakers.
Profile of NJ's Uninsured Population-CSHP is developing detailed population-based information on the health insurance status of various groups according to their demographic characteristics, economic health status, residential locale, and employment status and job characteristics using the NJ Family Health Survey and other data.
Assessment of Access to Affordable Health Insurance in New Jersey-
CSHP is drawing on several sources of data on health insurance premiums and out-of-pocket health insurance costs to assess the degree to which coverage affordability is likely to be an issue for the uninsured and privately insured populations.

A Steering Committee comprised of senior officials from the Office of the Governor, DHS and the Departments of Health & Senior Services, Banking & Insurance and the Treasury was convened to oversee all grant related activities. This grant has been extended, and all research activities outlined above will be completed by June 2004.

Racial/Ethnic Disparities

County Data Resources for Improving Health of Minority Populations
Office of Minority and Multicultural Health, New Jersey Department of Health and Senior Services

This pilot study will identify sources of race/ethnicity information in health-related data that are routinely gathered by local agencies, and will assess the usefulness of these data for tracking and improving minority health.

Pilot Study of a State Policy to Reduce Racial and Ethnic Disparities in Cardiac Care
Agency for Healthcare Research and Quality, U.S. Department of Health and Human Services
As part of CSHP’s BRIC initiative (see Building Research and Infrastructure and Capacity, under Health Data and Information), the Center is conducting a pilot study of a New Jersey initiative to encourage hospital cardiac catheterization units to improve outreach and access to cardiovascular procedures among the state’s minority residents.

Health Care Performance Measurement

Assessing Hospitals’ Use of Mandatory Medical Error Reports for Quality Improvement and Error Reduction
National Patient Safety Foundation

The project will assess how New York hospitals collect, respond to, and utilize state-mandated incident reporting for internal patient safety improvements and error reduction. This exploratory research will help identify ‘best practices’ as well as hospital characteristics associated with the use of New York’s mandatory hospital error reporting systems for internal improvement activities. Recommendations regarding system design, training, and support to improve the effectiveness of incident reporting systems will be included in the final report.

Data Analysis for the New Jersey HMO Performance Report Card
New Jersey Department of Health and Senior Services
The Center is analyzing performance data reported by Health Maintenance Organizations (HMOs) serving private businesses in New Jersey.The New Jersey Department of Health and Senior Services will use the results of these analyses for the annual New Jersey HMO Performance Report Card and other Department reports.

Pharmaceutical Policy

State Pharmacy Assistance Programs: State of the Art and Implications for National Health Policy
The Commonwealth Fund

Based on surveys of state pharmacy assistance programs, CSHP will produce tracking reports describing the program features such as eligibility rules, consumer costs and reimbursement policies and tracking trends in program expenditures, utilization, and enrollment . In separate reports, CSHP will also investigate selected state drug discount initiatives programs and pooled purchasing efforts. These reports will describe the evolution of these programs, variations in approaches, legal and operational challenges, and perceived effectiveness in reducing costs to consumers and states.

Impact of State Budget Pressures on State Pharmacy Assistance Programs
The Kaiser Family Foundation

This study will assess how state pharmacy assistance programs for low-income elderly and disabled are being modified, if at all, in response to increasing state budget pressures. Based on survey data from all state pharmacy programs and three case studies, CSHP analysts will assess what cost-containment strategies states are implementing, states’ processes for choosing the best approach for lowering costs, the savings achieved as a result, and the impact on consumers, pharmacies and manufacturers.

State Health Data and Information

New Jersey Family Health Survey
The Robert Wood Johnson Foundation

A representative sample of 2,264 New Jersey families was interviewed in 2001 and early 2002 to gathered data on health care coverage, access to care, health status, health service utilization, awareness of and enrollment in state programs, long-term care needs and caregiver burden, as well as demographics and employment status. To enable in-depth analysis of vulnerable populations, the survey over-sampled low-income families (under 200% of the federal poverty level) and those with members age 65 or older.
Findings will be available beginning in the fall/winter 2003.

New Jersey State Physician Census
The Robert Wood Johnson Foundation

This survey of over 8,000 physicians licensed to practice in New Jersey grew out of the recommendations of the NJ Commission on the Physician Workforce in 1999. The Census was conducted with the endorsement of the NJ Board of Medical Examiners (BME), the Medical Society of NJ (MSNJ) and other leading organizations interested in physician workforce policy. The Census includes basic demographic and practice information, and analyses are planned to assist in the identification of medically underserved areas and populations and to address other state policy questions.
Findings will be available beginning in the fall/winter 2003.

Building Research Infrastructure and Capacity (BRIC)
Agency for Healthcare Research and Quality, U.S. Department of Health and Human Services
Researchers at CSHP and other units of the Rutgers Institute for Health, Health Care Policy and Aging Research are building partnerships with other health services researchers and stakeholders in the state to improve New Jersey’s health services research capacity and make the state more competitive in obtaining federal health research funds.
